$140 per 45-minute session for Individual and Family sessions
$60 per hour of Professional Supervision services for Individual Session**

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health/behavioral health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is my co-pay per session? Is approval required from my primary care physician?
If I am a participating provider in your insurance plan, you will be responsible for payment of the co-pay, co-insurance or payment toward reaching your deductible at the time of service. Currently, I am an in-network provider with United Behavioral Health/OPTUM, Highmark, Capital Blue Cross insured clients.
I am unable to accept Medical Assistance clients.
Employee Assistance Plan (EAP)
If your employer has an EAP, you may be covered to receive services. You are responsible for checking with your insurance prior to beginning counseling services.
Typically this type of service allows you, an employee, to gain several sessions for free, under their provider list. The only information reported to your employer is the number of individuals serviced, their names and information are kept confidential. I am a participating provider with Mazzitti and Sullivan and Lifesolutions/Workpartners.
If you choose to continue services after the initial free sessions included on your plan, you will be responsible for the fees or may seek coverage under your health insurance plan.
Reduced Fee
Reduced fee services are available on a limited basis and for some clients needing to pay out of pocket or experiencing financial difficulty.
Payment
Cash and checks accepted for payment. FSA or HSA funds may be used to be reimbursed for counseling services, please contact your plan prior to utilizing this payment feature.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the session. Emergencies and inclement weather will not incur fees. This payment will be required prior to your next scheduled appointment. Your insurance company will not pay for "no show" appointments. Three "no show" appointments will result in client being discharged from Counseltations and a referral for other providers will be given to the client.
Please remember, this time is being set aside to help you reach your goals and before making appointments it is encouraged to check your calendar and try to keep all appointments scheduled.
